Once a month, you’ll be prompted to complete a short check-in form.

This is your moment to pause, reflect on how things are going, and be intentional about what comes next. The questions and timing may vary, but the purpose is always supportive, not evaluative.

To complete your check-in

  1. On the date the check-in is assigned, it will appear on your Dash as a task. (If that's not today, tap the calendar icon to navigate to the correct date)
  2. Tap the task to view and complete the form.
  3. If you’re not ready yet, tap Skip in the top right corner and come back later.
  4. Answer the questions (none are mandatory, so you can share as much or as little as you like).
  5. Tap Submit when you’re done. The task on your Dash will be marked as complete

When you hit Submit, the task on your Dash will be marked as complete.

If you tap on it again later, you’ll be able to view your responses.

Once you submit, I’ll be notified and can review your responses.
